How to Register
Complete the online registration form using the link below
New Patient RegistrationRegistration Medical
Once registered, make an appointment with our nursing team for a registration medical ONLY if you have never been a patient in the country before. If you have transferred from a practice within the UK we will have access to your records and a new patient registration is not required.
SCR – SUMMARY CARE RECORD
When we process your online registration, your Summary Care Record (SCR) will be set to “Express consent for medication, allergies, adverse reactions and additional information.”
If you do not wish to have an SCR, please complete the SCR Patient Consent Preference Form and email it to the surgery @ admin.bedfordpark@nhs.net
If you cannot register using the above link you may need to come in to the surgery and complete the paperwork at the reception desk. You will be provided with the relevant form at that point.

Temporary Residents
If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.
To register as a temporary resident you will need to complete a Temporary Resident Form. Please return the completed form to the surgery.
You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.

PATIENT REMOVAL FROM THE PRACTICE
Patients will be removed from the practice for the below reasons in line with practice policy:
- Movement out of the practice catchment area – 30 day notice provided to find a new GP
- Crime and Deception or Violence / unacceptable behavior – immediate removal if police are involved
- Irretrievable breakdown of Doctor / Nurse – patient relationship – 8 day removal request
- Embarkation – when a patient has moved out of the UK for more than 3 months – 30 day notice provided before removal
