MONKEYPOX AND COVID SCREENING FOR FACE TO FACE APPOINTMENTS

Dear Patient

In line with infection control procedures you will be asked the following questions whenever you book in for a face to face consultation with the clinicians at the surgery, for the safety of both the staff and the patient.

All the patients attending appointments must be screened prior to coming into practices. The screening questions are

  1. Are you aware you have been a contact of a confirmed case of Monkeypox in the last 21 days?
  2. Have you got 1 of more of the following symptom’s- rash, fever any swollen glands
  3. Have you got 1 or more of any respiratory symptoms including cough, fever or loss of smell/taste, sore throat etc.
  4. Have you received a letter from NHS stating that you are vulnerable or have any underlying condition that increases your risk of developing severe COVID-19 infection?
